02 · The handover

How the arc hands you along.

Same four stages as every Clearwater plan — here is who is in the room at each one, and why you never repeat your story.

Stage 01

Assess

Sixty minutes: full history, a movement screen, baseline numbers on the chart. You leave with a working diagnosis, not a guess.

In the room: the discipline your condition calls first — the other three read the result.

Stage 02

Plan

A written plan names the goal, the session count, and the re-test date — priced before it starts and shared with your whole care team.

In the room: your lead clinician. On the file: all four.

Stage 03

Treat

Hands-on treatment plus graded loading, drawn from whichever disciplines the plan calls for. One file, so you never repeat your story.

In the room: whoever the plan names — one visit can hold two disciplines.

Stage 04

Maintain

A taper, a home program, a check-in already on the calendar. Discharge is a milestone we plan for, not a day you fall off a cliff.

In the room: less and less of us. That is the point.

Every active file is reviewed at the Tuesday case meeting — four disciplines around one table, your numbers on the screen. If your plan needs a different mind on it, it gets one without you re-telling your story.
